Welcome to AZ Nurse Practitioner (Kristen Stout, FNP-C PLLC). By using our website, booking services, or opting into communications, you agree to the following Terms of Service. Please read them carefully.
1. Services Offered
AZ Nurse Practitioner provides health and wellness services including, but not limited to:
Bioidentical Hormone Replacement Therapy (BHRT)
Medically supervised weight loss programs
Aesthetic treatments including neurotoxins (Jeuveau)
All services are provided by licensed medical professionals. Eligibility and treatment depend on medical history and lab results, as determined by the provider.
2. Appointments & Cancellations
Appointments may be scheduled via phone, email, or online. We ask that you provide at least 24 hours’ notice for cancellations or rescheduling to avoid a cancellation fee. No-shows or late cancellations may be charged in full at our discretion.
3. Telehealth Services
Telehealth consultations are available for eligible services. By participating in a telehealth appointment, you consent to receive healthcare services via secure video or phone.
4. Payments & Refunds
Payments are due at the time of service. We accept major credit cards and HSA/FSA payments where applicable. All purchases are final unless otherwise stated. No refunds will be issued for services already rendered.
5. SMS & Email Communications
By providing your phone number or email and checking the opt-in box on any form, you consent to receive communications from AZ Nurse Practitioner related to your appointments, health services, and promotional offers.

6. Consent to Treatment
By becoming a patient of AZ Nurse Practitioner, you acknowledge that you have provided accurate medical information and consent to treatment plans recommended by your provider.
